About the Infant and Toddler Home Visitation Program

The Infant and Toddler Home Visitation program (ITHVP) utilizes the renowned evidence-based Parents as Teachers (PAT) curriculum and provides biweekly personal visits, either in-person or virtually, based on client preference, for up to 24 months.

The program partners with and receives referrals from several community-based organizations, including the Bridges Maternal Child Health Network, MOMs OC, Help Me Grow, Family Resource Centers, and Family Support Network. Additionally, any family living in Orange County with infants and children up to 5 years of age may contact the program to request services.

Program Elements

This program focuses on four key elements to ensure families have the support they need to raise healthy children and foster a nurturing family environment.

Biweekly home visits

Families receive biweekly home visits (either in-person or virtual) focused on child development, parenting strategies, and ensuring a healthy family environment through the Parents as Teachers (PAT) curriculum.

Parental guidance

The program offers guidance on child development milestones and positive parenting techniques, helping families to strengthen their caregiving skills and support their child’s growth.

Case management

Families are assisted by home visitors in accessing vital community resources such as healthcare, mental health services, and childcare, ensuring comprehensive support.

Long-term support

The ITHVP program helps families build a foundation for long-term success, providing tools and resources to foster child development and maintain a safe and nurturing home environment.

Program Timelines

The Infant and Toddler Home Visitation Program offers regular support for families, ensuring a consistent approach to child development and family wellness over the two-year program period.

Referral

Referrals are accepted from community organizations such as Help Me Grow, Family Resource Centers, and other partners. Families can also self-refer if they meet eligibility criteria.

Initial Contact

Once a referral is received, the program contacts eligible families within one business day to schedule the first home visit.

Biweekly Home Visits

After enrolment, biweekly home visits (in-person or virtual) are scheduled for up to two years. Each visit includes education using the Parents as Teachers (PAT) curriculum, focusing on child development and family well-being.

Ongoing Support

Home visitors work with families throughout the program to ensure they have access to community resources, such as healthcare, childcare, and mental health services.

Program Conclusion

At the conclusion of the two-year program, families are equipped with the tools, education, and resources to continue fostering their child’s development and maintaining a healthy family environment.
