Program Elements
Our approach focuses on four core elements that help individuals and their families navigate the crisis and transition to long-term support.
Crisis Intervention
Provides immediate, field-based crisis intervention within 75 minutes of referral, offering support to adults at imminent risk of psychiatric hospitalization.
Intensive Therapy
Delivers in-person, intensive therapy services over a 3-week period to stabilize mental health crises and prevent out-of-home placement.
Case Management
Offers comprehensive case management to assist clients in accessing resources and connecting to long-term care and support services.
Family and Peer Support
Provides family support and peer specialist services to enhance the client’s support network and promote sustainable recovery.
Program Timelines
The Adult In-Home Crisis Stabilization Program follows a structured timeline from intervention to support during the 21-day stabilization period.
Referral
Referrals are accepted from authorized sources via fax, phone, or email. The program team reviews the referral and schedules the intake based on the client’s situation.
Initial Response
A clinician responds within 75 minutes to conduct an intake and initiate crisis intervention for emergencies. Non-emergency cases are scheduled as quickly as possible.
Immediate Support
The program team continues to provide immediate short-term therapy and interventions to help stabilize the client’s mental health, involving the family where appropriate.
Ongoing Service and Resources
Families are connected to long-term services and community resources to ensure ongoing support after the crisis period.
