About the ECM Program
Enhanced Care Management (ECM) is a California Medi-Cal benefit designed for youth and provided through their managed care plans. This no-cost, personalized program is offered through The Priority Center to help connect all aspects of a member’s care to improve their overall health and well-being. It provides intensive, person-centered care management that addresses both medical and social needs simultaneously.
Audience
This program is specifically for Medi-Cal members under age 18 who are dealing with mental health challenges or chronic conditions. It is ideal for families who may not have a primary care doctor or those who need a high level of support to manage complex medical and social needs.
Eligibility
To qualify for ECM through The Priority Center, youth must be under the age of 18 and enrolled in a Kaiser Medi-Cal managed care plan. Additionally, they must meet at least one of the following criteria:
- Experiencing homelessness or unstable housing.
- Frequent ER or hospital visits.
- Significant behavioral health needs.
- Current or former foster youth or child welfare involvement.
- Transitioning out of incarceration.
What We Address
Our program is designed to manage complex challenges that impact a child’s health, including:
Chronic Health Conditions:
Proper medical care for high-risk physical and mental health conditions.
Behavioral Health:
Support for significant mental health or substance use needs.
Social Barriers:
Identifying and resolving needs related to housing, food, and transportation.
Symptom Management:
Preventing the escalation of symptoms by ensuring members receive prompt, quality care.

Program Elements
Members are supported by a compassionate team of healthcare professionals and mental health advocates through the following elements:
Lead Care Manager
A dedicated partner who assesses your needs and creates a customized care plan to help reach wellness goals.
Care Coordination
Assistance with finding doctors or psychiatrists and scheduling medical appointments.
Resource Linkage
Connecting families to community resources and helping them enroll in eligible benefits.
Advocacy
A full care team that coordinates housing and treatment plans while acting as all-around advocates.
Voluntary Participation
Members can opt out at any time and will not lose their regular health benefits by joining.
